Description This assessment tool is intended for use by music therapists working with older adults in skilled nursing facilities. In particular, the tool is based on (and shares assessment domains) with the Minimum Data Set, part of the federally-mandated (United States) assessment process for recipients of Medicare and Medicaid residing in nursing facilities. The assessment protocol involves interviewing, observational methods, and musically-interactive exercises to assess recipient functioning and to plan appropriate therapeutic interventions.
